Participant Details


CONSENTS

I consent to   Australian Behaviour Consultants Pty Ltd (ABN: 94 650 644 465)  trading as   ABC Behaviour Support (ABC)  to be endorsed with the NDIA as "My Provider" for the category: Behaviour Support and Improved Daily Living (Organisation ID: 405 011 7479).

This consent applies throughout the intake period and will remain in place for the duration of any subsequent formal engagement with ABC Behaviour Support.

How to change my consent

This consent will remain in place until you withdraw. Consent can be changed at any time by:

  • Submitting a new consent form:   Consent Form

  • Ending this service agreement and contacting the NDIA (   1800 800 110) to update provider endorsement.

Australian Behaviour Consultants Consent

I give permission for ABC to conduct assessment activities and write reports i.e., Behaviour Support Plan or therapy report for the participant. I give my permission for ABC to:

  • Read my/the participant’s records. Observe me/the participant.

  • Speak with my/the participant’s support team.

  • Train my/the participant’s support team in the Behaviour Support Plan.

  • Use a photo of me/the participant or my/the participant’s environment, in reports about me.

  • Record and review video of me/the participant in their environment.

I understand that the purpose of these activities is to gather information about me/the participant for the development of reports, i.e., behavioural data collection, medical history, and incident reports that will assist in developing a more thorough report and/or plan. I understand Behaviour Support Plans may contain recommended restrictive practices and I am able to withdraw this consent at any stage.

Consent to share information

I am the person/person responsible for the above-mentioned NDIS Participant and hereby give permission for ABC to exchange/release any or all information pertaining to me/the participant. The following is not an exhaustive list of example organisations etc. with whom ABC may exchange/release information:

  • NDIS service providers such as my/the participant’s accommodation provider, support workers, and day program service providers.

  • NDIS therapists such as my/the participant’s occupational therapist, physiotherapist, speech therapist.

  • Allied Health Professionals such as my/the Participant’s General Practitioner, Psychologist, hospital staff. NDIS Quality and Safeguards Commission.

  • Department of Communities and Justice (DCJ).

  • NSW Department of Education (or other education departments).
